Your First Visit

When you come for your appointment, please remember to obtain and bring the following:

• Driver's license or a valid ID
• Insurance information
• Referral letter (if required)
• Copies of results, X-rays, MRIs, CT scans, etc. and any other relevant information
• List of medications (if any)
• Workers Compensation / Auto claim number / Adjuster name / Phone number - Authorization prior to your appointment is required

Failure to provide above listed information required by KOC and your insurance, prior to your appointment may result in your appointment being rescheduled.

If patient is a minor (17 or under), and a parent/guardian is unable to accompany the patient to their visit(s), please complete form "Permission to Accompany a Minor" in addition to new patient paperwork.
